First:
Attempt to locate the individual's NRHS Membership Card. By
looking at the card, identify the
membership number AND class of membership (At-Large or Chapter).
To
reporting the death of an At-Large member:
Write,
fax, or e-mail, the National Office.
You must provide the member's name, address, date and place
of death, and membership number (if available). You must also
include your name, address, and phone number so we can contact
you if we have questions or to verify the information provided.
We may require written documentation.
To
reporting the death of a Chapter Member:
Contact
the individual's local Chapter
of the NRHS. You must provide the member's name, address, date
and place of death, and membership number (if available). You
must also include your name, address, and phone number so the
Chapter can contact you with questions or to verify the information
provided. The Chapter may require written documentation. See
the next section also.
If
the individual has additional Chapter Memberships in one or
more additional NRHS Chapters besides the Chapter where the
Regular Membership is held:
Contact
each Chapter of the NRHS
in which you can determine they had a membership. You must provide
the member's name, address, date and place of death, and membership
number (if available). You must also include your name, address,
and phone number so the Chapter can contact you if with questions
or to verify the information provided. The Chapter may require
written documentation. You must individually report your change
to each Chapter.
If you
are a Chapter Membership Chairman and wish to report the death
of a member:
Write,
fax, call, or e-mail, the National Office.
You must provide your membership number, your name, and your phone
number, your Chapter's Name, as well as the member's name, old
address, new address, and membership number. We may contact you
to verify the information provided.
